Enrollment & Referral Information

This enrollment application and referral process are NOT intended for those seeking crisis support. These submissions are not monitored 24/7. For crisis support, please call OBHC at (509)826-6191, the toll-free OBHC line at (866)826-6191, or Crisis Connections at (800)852-2923.


Enrollment Application

This form is for individuals seeking voluntary enrollment in one or more of OBHC's services for themselves, their children under the age of 13, or other individuals under your legal guardianship.

This form is not intended to be completed by anyone other than the individual seeking service(s) or their guardian (for children under the age of 13).

If you are referring an individual for service, please instead follow the instructions found under “Referrals”.

Upon successful submission, your Enrollment Application will be reviewed and processed by a staff member within 1-5 business days.

Once processed, you will be contacted by a staff member. Our staff will confirm your application status, work with you to identify any missing information, and instruct you on what to expect during the remainder of the enrollment process. 

Referrals

We accept referrals for our services from individuals and providers.

With few exceptions, services at Okanogan Behavioral HealthCare are voluntary.

This referral form is intended to be utilized for individuals who are willing and interested in recieving services from OBHC. Please ensure the individual knows you are referring them for services.

To refer someone for services - download and complete either the adult or child/adolescent referral form and submit the form to OBHC via fax or email.

Due to the confidential nature of the services provided by OBHC, we can not share information with you about the individual you are referring for service(s). We can confirm that we recieved your referral, but we can not give you any information on our contact with the individual, their enrollment status, or potential treatment. Information will only be shared with individuals and/or organizations listed in the client’s ROI forms.
